The HAP was established in 2007 by the Jefferson County Department of Health

The goal is to improve community health by putting into practice what citizens have identified as needs
for themselves, their families and their neghborhoods.
To support this work - more than $13 million has been awarded Jefferson County through the
CCPW (Communities Putting Prevention to Work) federal program.
THANK YOU!!!
To the Community Foundation of Greater Birmingham and the United Way of Central Alabama

hundreds of non-profit programs endeavoring to make our community a better place
to live and work each and every day!
